Community Trigger Referral Form

 
Please use this form if you have already reported antisocial behaviour (ASB) on three separate occasions relating to the same ASB issue or concern in the last six months OR the issue or concern affects more than five households or business premises and you’re requesting review of that process.
 
In an emergency please contact the relevant emergency service, police, fire or ambulance on 999.
 
Please complete this form as fully as possible. All information is treated in the strictest confidence. However, we will need to share information with our partner agencies in order to provide details on whether the threshold has been met to activate the community trigger. To ensure this happens the information provided is collated accordingly and a case will be created on ECINS, this is a case management system used to record and share information. Your case may be passed to an appropriate lead agency who will respond to you such as the police, your housing provider or public health and housing team.
 
It is important to both you and us, that you are kept informed about the progress of your referral.
To ensure this is achieved once your referral has been completed and submitted we will send you an email acknowledging the receipt of the referral form within 3 working days. The next part of the process will involve a case review taking place and a response being sent to you detailing whether the threshold to activate the community trigger has been met, this will take place within 10 working days of the acknowledgement.
